Inclisiran: A Novel Agent in the Management of Hypercholesterolemia
PCSK9 is a serine protease that binds to the LDL receptors and targets the receptors for lysosomal degradation, thus reducing their recycling and decreasing the removal rate of circulating LDL-C. Monoclonal antibodies directed against PCSK9, such alirocumab and evolocumab, have been shown to cause a significant reduction in LDL-C levels and improve cardiovascular outcomes. On the other hand, inclisiran is a chemically synthesized siRNA molecule, which lowers LDL-C levels by targeting the hepatic production of PCSK9, as it prevents its translation via degradation of PCSK9 mRNA post-transcription. This review aims to discuss the current evidence regarding the role of inclisiran in the management of hypercholesterolemia.
